Medical Social Worker

Position

(Full Time)

The Medical Social Worker shows sound knowledge and proficiency in the use of
social casework, theory, methods, and techniques and of their application to cases involving medical,
social, and psychiatric social problems. This position reports directly to the Director of Case
Management and Social Services. The Medical Social Worker assumes responsibility and accountability
for the duties and is cognizant of the philosophy, standards, objectives, and policies of the Schneider
Regional Medical Center and the Health Information Management Department.

Education & Experience

Bachelor’s degree in social work or a related field, Master’s Degree
preferred.

Work in this category is majorly sedentary and involves
sitting, some standing and walking; employees may be required to lift or pull up to 10 lbs.
frequently or 20 lbs. occasionally

Duties & Responsibilities

• Be able to make comprehensive Psycho- Social Assessment and make appropriate
referrals throughout the age continuum.
•  Have a broad knowledge of Community Agencies and Programs, being able to make the
appropriate referrals to them, rehabilitation and treatment programs, home health care
agencies and in system resources.
• Have broad knowledge of Pediatric population and laws related to abuse. Be able to
assess for abuse and refer to appropriate Agency.
• Broad knowledge of Geriatric Population is able to assess for abuse and refer to the
appropriate Agency.
• Knowledge of how to complete an off-island transfer/ with and without insurance and
Medicaid.
• Knowledgeable of and accurately completes appropriate forms needed for Referrals i.e.,
Medicaid, Dial –A- Ride, Vitran plus etc. and knows criteria for each.
• Maintains knowledge of the Hemodialysis population and process for
Transient/Permanent transfers In and Out.
• Must participate as an integral part of the Medical/Surgical and Psychiatric Treatment
Team and participate in any requested meetings i.e., Family meetings, Bio-Ethics
meeting, Utilization Review Meeting etc.
• Communicates clearly and accurately in both in written and verbal communication.
• Facilitates the Case Management processes; works closely with Nurse Case Managers.
• General knowledge of Psychiatric population; knows commitment process and able to
facilitate same.
• Functions within guidelines and transfer process to meet all regulations required for an
effective transfer to a Skilled Nursing Facility.
• Demonstrate effective counseling skills.
• Be able to identify and analyze from a proactive plan of care, working to eliminate
system problems that create the variances in care.
• Demonstrate the ability to address social issues in all areas of the hospital.
• Demonstrate a thorough understanding of various payer issues relating to covered
services and contracted agencies.
• Participate in professional growth through seminars, workshops, conferences,
publications, and professional affiliations.
• Understand the purpose and function of utilization management and collaborate with
the utilization management process.
• Acts as a resource to patients, families, physicians, nursing staff and the community.
• And all other duties as assigned.
