Medical Social Worker (Hemodialysis)

Position

(Part Time)

facilitates the transplant process and establishes relationships with procurement organizations to ensure patients accessibility and preparedness for transplantation; coordinates referrals and required testing and evaluations for patients in preparation for transplantation; facilitates transient and permanent transfers, performs mental health and psychosocial evaluations and support for all dialysis patients and donors; works collaboratively with the Unit’s interdisciplinary team.  Performs other duties within the scope of practice as deemed necessary in the provision of care.

Education & Experience

Any combination of a Master’s degree in Social Work or related field from an accredited program along with a minimum of three years of social work experience; License in Clinical Social Work and current VI licensure required; a minimum of two years’ experience working with patients with renal failure preferred.

Hazards & Physical Requirements

Performing the duties of this job requires sitting, standing, walking.  Hazards include potential exposure to blood and body fluids and communicable diseases.  Working under stressful conditions and irregular hours.

  • Ability to communicate effectively orally and in writing with clients, customers of client, and staff.
  • Ability to be flexible based on patient and the Department’s needs.
  • Possesses strong computer literacy skills.
  • Knowledgeable of dynamics of kidney failure, transplantation and post transplantation requirements.
